1. Data Privacy & Information Security:

At Mayhem Tattoo, LLC, we value your privacy. We collect personal information solely for the purpose of scheduling, safety, and legal compliance. We will never sell or share your personal information with third parties unless required by law or necessary for the processing of your appointment.

2. Independent Artist Structure:

Except for our staff artist (Katie), all artists at Mayhem Tattoo operate as independent subcontractors. Each artist is a separate business entity. Your personal data and design ideas are shared with your specific artist only upon your confirmation and booking.

3. Pricing & Payment Policies:

Because our artists are independent, they are solely responsible for their own creative work and pricing structures.

• Written Estimates: To ensure transparency, please ensure you receive your pricing in writing on your Tattoo Release Form before the session begins.
• Stencil Approval: You must approve and initial your stencil design and placement before any tattooing starts.
• Deposits: Deposits are required to secure your time. Please refer to our Booking Policy for details on non-refundable terms and rescheduling.

4. Dispute Resolution

We strive for 100% customer satisfaction. If you have concerns regarding a design or pricing, please address them directly with your artist before or during your appointment.

5. Consent:

By booking an appointment and signing our Tattoo Release Form, you acknowledge that you have read and agreed to these privacy terms and studio policies.
